How to Register with Practice

Please enquire at reception, if you live in the catchment area (Deeside) and are currently not registered with a G.P in the area, please complete a registration form. To complete this form you will need your NHS number and proof of identity.

*It is the responsibility of the Patient to notify the surgery if they are unable to keep an appointment
